The above dog is being adopted solely as a pet for myself and/or immediate family. I agree that I will not sell, give away or otherwise dispose of said dog to any person(s), shelter, dealer, retailer, auction, institute or any other entity for any reason. If, at any time I am unable or unwilling to keep this dog, I agree to immediately return him/her to Angie's Stray Foundation, via the foster family from whom I adopted from, or via www.angiesstrayfoundation.org*
I hereby agree to care for the above dog in a humane and responsible manner and to provide him/her with clean and adequate shelter, food, water, and veterinary care. I further agree that said animal will mainly reside in my home and be outside properly supervised and/or restrained with a fenced in yard to protect them from being lost or injured.*
I understand that many dogs are uncomfortable, even frightened, around children, so particular care will be provided if children are interacting with the adoptive dog. I accept full responsibility for all such circumstances and any problems that may occur.*
I understand that Angie's Stray Foundation has provided appropriate veterinary care, including care for illness such as URI, vaccinations as appropriate, and spay/neuter if the dog is of age (see next point). Regardless, I will introduce my new pet to my veterinarian within a month after adoption.*
If the dog has been adopted prior to being spay/neutered, you promise to ensure that spay/neuter will be done at the age appropriate to the breed. A refundable deposit may be kept until such time proof of spay/neuter is received.*
If the dog has been adopted prior to being of age to receive the rabies vaccine, you promise to ensure that proper vaccination will be completed at the appropriate age.*
I hereby agree to have the dog examined annually by a licensed veterinarian, kept current on all recommended vaccinations, and licensed in the municipality in which the animal resides.*
I hereby agree to retrieve the dog from any public shelter if notified he/she is there, and to contact Angie's Stray Foundation immediately if the animal is lost, stolen or dies.*
I hereby agree that a Angie's Stray Foundation representative may visit the dog at any time after the adoption to evaluate his/her condition and environment, and may confiscate the animal if living in unsuitable conditions, neglected, or otherwise abused. Failure to abide by this agreement is also grounds for confiscation of the animal with no refund of adoption fee*
I understand that Angie's Stray Foundation makes no representations or warranties, expressed or implied, about the animal’s health. I am aware that this animal may have been exposed to contagious or infectious disease or may have been injured prior to rescue by Angie's Stray Foundation. Despite veterinary care already provided and Angie's Stray Foundation best efforts to ensure he/she is healthy and sound, unknown conditions may exist. Angie's Stray Foundation will in no way be responsible for any health problems that may arise. I furthermore agree to care for the dog should he/ she develop a health problem, and I understand that veterinary care can be expensive.*
I understand that many animals require time to adjust to a new home, regardless of whether other animals and/or children are present. Exercise prudence, caution and patience when introducing an dog into a new environment. Angie's Stray Foundation is available to assist with introduction and/or any adjustment issues.*
I understand that negative and even damaging behaviors may arise in the future and commit to working with the dog utilizing obedience training and techniques to ensure harmony and years of love and happiness within your family*
I understand that Angie's Stray Foundation makes no representations or warranties, expressed or implied, about the animal’s temperament. Angie's Stray Foundation is hereby absolved of any liability and shall not be held responsible for any bodily injuries, property damage, or other damage the adopted animal may cause. The adoptive owner agrees to hold the Foundation and any of its representatives harmless from loss and expense, including legal fees, due to placement of adopted animal.*
I hereby acknowledge that while I may surrender the dog/puppy back to Angie's Stray Foundation, I will not be receiving a refund. The donation will for the Foundation for the care and well-being of all of the animals in our care.*
I hereby acknowledge that I have read, understood, and agree to the terms of this adoption agreement. I have received all medical records and additional information pertaining to the animal. As of the date of this adoption, I will assume full financial responsibility for the above-described animal and will keep said animal as part of my family.*
Know all persons by those present (foster/agent for Angies Stray Foundation, Inc.), that for and consideration of the Adoption Agreement of the parties dated (todays date):

In return for valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the "Adopter" whose name appears below, does hereby: Release, remise and forever discharge Angie's Stray Foundation, Inc., a Pennsylvania Corporation, its foster families, volunteers, employees, agents, subcontractors, successors, and assigns, of, against, and from any and all claims, demands, damages (either direct, indirect, incidental or consequential), costs, expenses, losses, actions and/or causes of action of any and every kind and/or nature whatsoever (both as to matters and things now known as well as to matters and things which may hereafter be discovered, if such there be) which the Adopter may now have or may hereafter have on account of any matters arising and/or events occurring as a result of the subject Adoption Agreement
