Foster Agreement & Liability Waiver
Assignment of animal (check the box to agree with this statement):

I hereby agree to provide a temporary foster home for kitten(s), cat(s), dog(s), and/or puppy(ies) as assigned by the intake/foster coordinator, or other authorized personnel. I understand that the time limit for foster care will be discussed and can vary, depending on the needs of the animal. I understand that the animal will remain in my home for the entire foster period, but that Angie’s Stray Foundation will maintain ownership until the animal is placed into its adoptive home. I agree to keep my family pets up to date on vaccinations while I am fostering animals through Angie’s Stray Foundation. I state that I agree to provide humane and loving care to the kitten/cat/puppy/dog(s). This includes food, water, shelter, socialization, and enrichment.
Medical/Veterinary Care (check the box to agree with this statement):

Any veterinary appointment requests (emergency, well-visit, or follow-up) will first be approved by Ruth Jenkins or the foster coordinator who assigns the foster to you. The treatment of acute or other illness is at the discretion of Ruth Jenkins and/or your foster coordinator, and this includes the nature, duration, location, and denial of the treatment. I agree to use Elk Creek Veterinary Services: 2236 Baltimore Pike, Oxford PA, 19363, or another facility with prior approval by Ruth Jenkins your foster coordinator. Angie’s Stray Foundation will assume the cost of customary and reasonable medical care to ensure the health of the pet. If I want to use my own veterinarian, I will assume the cost of care.

While you are caring for foster pets, we will contact you from time to time to check on the pets’ progress and address any concerns you may have. Please contact Mia Vella for any needed supplies. Crates or other fostering items can be lent to fosters and must be returned to Angie’s Stray Foundation when the foster animal is returned or placed in a forever home. At any time, you are in need of assistance please reach out to Ruth Jenkins and/or your foster coordinator directly, or seek advice via ASF Kittens or Dogs Chat on Facebook Messenger. As a reminder any *emergency advice* must be received from Ruth Jenkins and/or your foster coordinator.
Quality of Life (check the box to agree with this statement)

To be a successful foster parent, you will need a compassionate nature, the cooperation of your family or roommates, flexibility, and some knowledge of animal behavior. Every measure will be taken to ensure a safe and comfortable living situation for the foster family and the foster animal. Because an animal’s temperament may change with its environment, Angie’s Stray Foundation cannot guarantee the temperament of any animal.
Termination of foster relationship (check the box to agree with this statement):

If for any reason the foster parent cannot fulfill their fostering duties, Ruth Jenkins and your foster coordinator must be notified, and foster parent must allow Angie’s Stray Foundation 3-5 days to find another foster home. If the foster parent decides to adopt the pet they are fostering, they must discuss this with the Adoption Coordinator and be subject to the adoption approval process and appropriate adoption fees. Angie’s Stray Foundation reserves the right to remove foster animals from a foster home at any time should it be deemed necessary.
Disclosure of Risk

I am fully aware that fostering any animal through Angie’s Stray Foundation has known and unknown risks. I acknowledge, agree, am aware of and assume the many risks associated with being around animals, including but not limited to the risks of being bitten, scratched, jumped upon, knocked over, chased, tripped, infected with a zoonotic disease, or otherwise injured or frightened, as well as the risks of damage to personal property. Angie’s Stray foundation cannot guarantee me or anyone else what type of temperament the animal(s) I am fostering may have. I hereby accept responsibility for the animal(s) in my care.
To Release from all Damages & Legal Action

I do hereby fully release and discharge Angie’s Stray Foundation, their officers, agents, volunteers, and employees from any and all claims from injuries, including death, which may accrue to me on account of my fostering a pet. I further agree to indemnify and hold harmless and defend Angie’s Stray Foundation, their officers, agents, volunteers, and employees from any and all claims resulting from injuries including death, sustained by me and arising out of, connected with or in any way associated with fostering an Angie’s Stray Foundation pet.
